I can't believe this tool - three years ago I remember discussing a similar idea with lecturers on my MA and we all decided it would be too tough to create....

There's a desktop-based tool called MediaNotes that allows you to annotate videos with both free text and tags, as well. It's available for both Windows and Mac.
